Health care survey being distributed

The Webb and Adiron-dack Communities Health Care Research Committee will be sending out a survey to taxpayers in Webb, Inlet, Long Lake (Raquette Lake), and Forestport (Woodgate, White Lake and Otter Lake). The purpose is to obtain public input about Health Care Services provided to our communities by the Town of Webb Professional Offices, and health care services that the public would like to have provided in the future.

The committee asks that you take a moment of your time and participate in this survey by completing it on-line at www.surveymonkey-.com/s/PSCSRWB. If this is not an option, the survey will be in the Jan. 24 edition of the Adirondack Express, you may return it by mail to Town of Webb Health Care Survey, PO Box 157, Old Forge, 13420, or drop it off at the Town of Webb Offices at 3140 State Route 28, Old Forge.  

Additionally they wish to update the community on relevant topics prior to the survey being sent out.

The current list of insurance providers accepted by the Central Adirondack Family Practice includes: Excellus Blue Cross-Blue Shield, Empire, RMSCO, GHI, Emblem Health, Univera, United Health Care, MVP, CIGNA, Medicare and Medicaid.

Patients covered by a Medicaid HMO must be seen by their primary care physician, or services will not be covered. Furthermore, no physician other than their primary care physician may bill such patients for services unless the patient was referred by the primary to another physician.

Except as noted above, anyone may be seen by the Central Adirondack Family Practice. If their insurance is not one of the above, they may wish to pay out of pocket for service and then submit a claim to their insurance carrier.

Walk-in patients may be seen at Central Adirondack Family Practice based on physician availability (there may be a long wait).

Individuals may be referred to another facility when the nature of the illness or injury is beyond the scope of treatment permitted at this type of facility, as governed by the State Health Department; or if the injury is job related, as the Central Adirondack Family Practice no longer handles Worker’s Compensation Cases.
