Lifestyle coaches will help prevent diabetes

Monday, May 05, 2014 - Updated: 9:16 PM

QUEENSBURY -- Adirondack Health Institute is partnering with CVPH Medical Center, Hudson Headwaters Health Network and Washington County Public Health to implement and expand diabetes prevention efforts in six Adirondack counties.

The Centers for Disease Control and Prevention National Diabetes Prevention Recognition Program launched in January in Clinton, Essex, Franklin, Hamilton, Warren and Washington counties. The New York State Health Foundation (NYSHealth) awarded AHI a grant to support this initiative.

"Adirondack Health Institute is both the facilitator and lead organization for this grant to build the infrastructure to develop a network of certified lifestyle coaches," explained AHI Director of Medical Home Initiatives Bob Cawley.

The initiative covers a largely rural population of about 300,000 residents, with some 13 percent living under the federal poverty level. The program will target those with pre-diabetes or who are at risk for diabetes.

"Although there are limited local health statistics available on patient population for both pre-diabetes and diabetes in our six-county region, we have higher obesity, smoking and high blood pressure rates and our patients may also likely be unaware of their risk for diabetes," said Cawley.
